3. National Insurance Number
4.  A digital copy of your passport photograph 
5. Disclosure and Barring Service (DBS)
6. Two proofs of current address
7.   PIN (Registered Nurses & Nurse Associates  only)NMC’s Statement of entry
8.  Proof of all qualifications, trainings (Certificates/diplomas)
9. Your CV if available.
